Access Forms
If you require access to PHRS Basic or Advanced View, please download the forms and submit to the eHealth Service desk.
To keep our system secure, PHRS View accounts that remain inactive for 12 months will be deactivated. If your PHRS View (Basic or Advanced) account is deactivated and you wish to regain access, you can contact the eHealth service desk team at servicedesk@ehealthsask.ca
and submit the required form (above).
